Applying to Long-Term Care

In Ontario, application and placement in Long-Term Care is managed through Home and Community Care Support Services. You can find the contact information for your local center at:

It is important that you go through the region that you currently live.

Home and Community Care Support Services will work through the application process with you and discuss homes that you have interest in.

Homes review each application individually to determine if they have the correct physical surroundings and talent ability to support the potential resident’s care needs.

Placement on the waitlist is determined by Home and Community Care Support Services based on the needs of the resident and urgency for placement into a home.

Once the application has been reviewed and accepted by the Long-Term Care Home, you will receive a letter of acceptance. This indicates that you are now on the waitlist for that home.

Home and Community Care Support Services will notify the prospective Resident once they have been matched to a bed, and process for admission.
